Attorney General Lisa Madigan announced today that a Cook County Grand Jury has returned indictments against Forest Park L.L.C. and Care Centers, Inc., the licensee and owner of the Pavillion of Forest Park nursing home.

Madigan said the indictments, returned on Sept. 7, charge the two entities as well as Jason Garti, M.D., a former medical director and wound care doctor at Pavillion, with multiple charges of Gross Neglect of a Long Term Care Facility Resident.

The Pavillion is located at 8200 W. Roosevelt Rd.

The charges are the result of an investigation by the Illinois State Police Medicaid Fraud Control Unit, according to a press release from Madigan’s office.

On Sept. 11, 2002, the Department of Public Health was contacted by emergency room personnel at Loyola Medical Center who said that a resident who had been transported from the Pavillion to the center had a large area of decubitus ulcers, commonly known as bed sores.

The indictments charge Garti with two counts of Gross Neglect of a Long Term Care Facility Resident, a class 4 felony punishable by one to three years in jail. Forest Park, L.L.C. and Care Centers, Inc. were charged with multiple counts of Gross Neglect of a Long Term Care Facility Resident.

If convicted, the release states, the charges may result in the closure of the nursing home.
